eABR on the Interacoustics Eclipse

 

Did you know that the Eclipse can perform eABR?

eABR is the measurement of the ABR (Auditory Brainstem Response) using an electrical stimulus rather than an acoustic stimuli. It is predominantly elicited via the patient’s cochlear implant.

There are many reasons why you may choose to perform an eABR, including to verify the position of the electrode array intraoperatively to, assisting with programming the sound processor for those who cannot reliably report comfort levels.

If you have got experience in traditional ABR techniques, eABR will be fairly straightforward to perform. Patient set up and preparation is exactly the same with the goal of low impedances and a settled patient state being ideal. The main factor to consider is reduction of excessive noise. This can often be problematic within operating theatres but not always impossible.

In addition to the Eclipse platform and software the following equipment is required:

  • Trigger cable to connect the Eclipse to the implant programming interface (Note: different trigger cables are required for the different CI manufacturers)
  • Cochlear implant programming software- this is to adjust the current level used to elicit the eABR.
  • Cochlear implant stimulator

 There are a few factors which will impact your eABR recording:

  • Stimulus polarity
  • Stimulus rate and level
  • Muscle artifact- PAM not to be confused with wave V
  • Artifact- power up of CI or coil interference

 Other important notes:

  • Earlier waves will often not be observed due to artifact
  • Latencies for eABR are shorter than traditional ABR due to direct electrical stimulation of the auditory nerve (i.e. bypassing the travel time from headphone to cochlear nerve). The latency is typically 1.5-2ms shorter
  • There is no shift in latency with an increase or decrease in stimulation level

                    

The Eclipse software comes with a default eABR protocol which will allow you to get started right away. Modifications can be made to the default protocol if required.
